Requests for Entire Medical Record/Specific Program/Test Results

Note: You must specifically request information on communicable diseases, Tuberculosis and HIV or it will not be included.
Instructions
Please submit your request along with a copy of your identification. The Michigan Medical Records Access Act 47 of 2000 (333.26265, Section 5g) requires identification to be provided with each request.

Please include the following information on your written request:

  • Date of Request
  • Patient's full legal name and/or previous names
  • Patient's date of birth
  • Requestor's name, phone number and relationship of the patient
  • Copy of your identification
  • Type of records you are requesting (Entire Record, Immunizations, Lead, Hemoglobin etc.)

Michigan Immunization Record

Kent County Residents

If you live in Kent County and only need a copy of your Michigan Immunization Record, you will be asked to go to one of our clinics to request your record. Please bring valid identification. If you are picking up a record for someone other than yourself or your child who is over the age of 18, you will need to bring a signed and dated note giving you permission to obtain the record. WIC Verification of Certification (V.O.C) for WIC Client Transfers

Please contact Kent County Health Department Clinical Services Administration WIC Department at 616-632-7200 or via fax at 616-632-7299 to request a copy of a V.O.C.
