Participants Rights & Responsibilities

Appointment Checklist

  • Verification of Certification (VOC) form or WIC Book
  • Proof of identity Example: Drivers license, Michigan or Student ID)
  • Proof of income (Household) (Example: Recent check stub, W2’s, unemployment paperwork, etc…)
  • Medicaid card(s)
  • Proof of residency (Example: Drivers License or utility bill)
  • The actual child or infant or mother who has the appointment
  • Verification of newborn birth (Example: birth record, discharge papers or crib card)
  • Proof of pregnancy (Example: ultrasound picture, prenatal vitamins, etc.)
  • Court documents (guardianship papers) if foster parent, grandparent or new custodial parent
  • Paperwork you may have from being on WIC before, including from another county or state

Note: If your appointment is just for a nutrition education class and to have benefits added to your WIC EBT card, you only need to bring your VOC or ID.
