Personal Action Toward Health (PATH) Workshop

The Personal Action Toward Health Workshop is a free community-based self-management program for adults with chronic disease (diabetes, heart disease, high blood pressure, arthritis, lung disease, etc). It has been shown to help people with chronic disease improve their health status, increase their ability to manage their health and maintain active, fulfilling lives.

The workshop is highly interactive. The mutual support and success of participants’ build their confidence regarding health management. The workshop sessions are led by two trained facilitators. At least one facilitator has a chronic condition.

The two and one half hour sessions are held once a week for six weeks. Topics covered include:

  • Ways to deal with frustration, fatigue, pain and depression
  • Exercises for maintaining and improving strength, flexibility and endurance
  • Appropriate use of medication
  • Strategies for communicating with health professionals, family and friends
  • Easy, effective nutrition tips
  • How to evaluate new treatments
